SPLENDID LEADERS SEMINAR To Register The registration fee is $450 and includes a continental breakfast, refreshment break and lunch. A $50 discount is available for more than one individual attending from the same organization. Please choose from one of the following to register for this seminar, either register online, fax or mail completed form to Splendid Leaders Seminar, 1270 North Fairfield Road, Dayton, OH 45432. Payment must be made by check payable to Universal Technology Corporation, or VISA, MasterCard, American Express credit card. Invoicing will be accepted only for Government Training Form DD-1556, which must accompany the conference registration form. Please note, if paying with a government IMPAC Card, it is not necessary to send the DD Form 1556 unless billing is required. Registration along with payment must be received by Friday, July 13, 2006. Seating is limited to the first 25 registrants.
Cancellation Policy Refunds must be requested in writing and received no later than July 13, 2006. Written cancellations received by this date will be refunded in full less a $50.00 processing fee. No refunds will be given after July 13; however, substitutions are welcome. All participants who have provided an appropriate email address will receive an email confirmation when the registration has been processed.
